Two chip components have shorted together during reflow soldering. In the first case the design rules are incorrect. Due to general placement errors this defect could occur. During reflow soldering some movement will occur potentially allowing shorts like this to form. In an ideal world on a perfect production process this may not occur, but designs should be robust to withstand the real world of manufacture.
